Community & Human Services (CHS) manages affordable housing, RSVP Volunteer Program and Human Services such as prescription reimbursements and Services for Seniors.

Affordable Housing Forms for Developers

•     Density Bonus Application (coming soon) - a form filled out by developers looking to add density to a proposed affordable housing development
•     Expedited Review Process (coming soon) - a form filled out by applicants for expediting the development review process for development that provides housing that is affordable.

Human Services Forms for Assistance

• Application for Prescription Assistance (coming soon) - a form for in need individuals seeking help to pay for medications
• Request for Services for Seniors (coming soon) - a form for in need seniors seeking help with various at home maintenance and health issues

RSVP Volunteer Forms

• Volunteer Registration Form - a form for interested individuals to fill out to become volunteers
