The State of Florida has been granted waivers of certain Title XIX requirements to provide special Medicaid services to specified groups of people. The Home and Community-Based Medicaid Waiver provides services to eligible aged/disabled individuals to promote, maintain and restore health or to minimize the effects of illness and disability. This is intended to delay or prevent institutionalization and allow eligible recipients to remain at home as long as possible.

Who should be referred?
Persons who are 60 years of age or older, who meet the income and asset criteria for Medicaid Waiver and who are functionally impaired to the extent that they are at risk of nursing home placement without in-home help.

Services provided include:
Case Management - A case manager is assigned to every client to assess the problems that the individual is experiencing and determine what services can best resolve those problems.

Support Services - Assistance with bathing and personal care, housekeeping, limited medical supplies, emergency alert response system, in-home respite, facility-based respite, and home-delivered meals and other support services as needed.

Funding
Funding is made available through the Area Agency on Aging, State of Florida Department of Elder Affairs, State of Florida Medicaid Program and the Collier County Board of County Commissioners.
